Service Content

Who provides it, and what do they do?

The team arranges suitable staff based on the needs identified; the actual service content, frequency and responsible roles will be confirmed before the service starts.

Physiotherapist

Assess mobility and arrange muscle strength, balance, endurance or walking training as needed.

Occupational Therapist

Assess self-care activities and the home environment, and provide training, assistive device or environment modification advice.

Speech Therapist

Assess communication or swallowing function as needed, and discuss the training direction with the individual and caregivers.

Registered / Enrolled Nurse

Provide nursing support according to professional assessment and service arrangement; specific procedures will be explained when the service is confirmed.

Registered Dietitian

Assess dietary, nutritional and weight needs, and provide dietary advice based on individual circumstances.

Rehabilitation Assistant

Assist in carrying out the established nursing or rehabilitation plan under the guidance of a nurse or therapist.

Health Worker

Provide companionship, personal care, escort, medical accompaniment or record-keeping support according to confirmed arrangements.
