One or two in-depth sessions, accompanied by a doctor and social worker. We first understand your views on health, values and family considerations, then explain the trigger scenarios, legal effect and options of advance directives in plain language, so the conversation stays meaningful rather than procedural.
02Document drafting
Drafting the formal document based on the discussion, signed by qualified witnesses. We also file a copy in the medical record so the attending team can access it when needed, avoiding repeated questions later.
03Ongoing follow-up
We recommend a yearly review or a review whenever health changes. We will proactively remind you of the review window, and when circumstances change, help update the document and notify family and the care team so your wishes stay close to who you are today.
